We are currently hiring a RN Clinical Care Manager based in Worcester MA. The Clinical Care Manager (CCM) provides, coordinates, and organizes evidence-based care management to our Medical Assistant Treatment Program (MAT) patients.

Salary Range: $35.00-39.00/Hour
Essential Functions
- Responsible for assisting Primary Care Providers (PCPs) in coordinating and managing the care of MAT program patients, ensuring they receive optimal care, education on lifestyle changes, and preventive care across multiple settings.
- Manages care plans for patients transitioning between healthcare facilities, aiming to prevent disease exacerbation, improve outcomes, and reduce hospital and ER visits.
- Identifies and coordinates care, maintaining communication with the care team on patient status and needs.
- Provides direct patient care, including assessments, vaccinations, medication reconciliation, and lab reviews.
- Involves patients and support systems in care planning and education. Assists in creating self-management plans and documents patient care and progress in the EHR. Evening, weekend, and holiday shifts may be required.
Education and Experience
- Registered Nurse License in MA
- Bachelor’s degree in Nursing from an accredited college or university
- 1-2 years of clinical experience
- Current CPR certification
