We are currently hiring a Reproductive Health Navigator based in Framingham. Participates as a member of the Reproductive Health care team, patients who have been diagnosed as having abnormal reproductive health diagnoses and require further diagnostic testing and follow-up. Assist patient with needed education and follow up appointments and will work collaboratively with other team members in providing patient care. Assist in coordinating care and additional diagnostic testing, follow-up for individual patients and maintain the cancer tracking database. This position is a licensed healthcare professional that can perform specific duties regarding reproductive health contraceptive injections, patient education and provider driven treatment for sexually transmitted infections, cancer diagnosis education, follow up, tracking and screening, and clinical support.

Salary Range: $20.00-24.00/Hour (Medical Assistant)
                         $ 26.00-29.00/Hour (LPN)

Essential Functions:

  • Provide direct patient care within the scope of practice including patient assessment, administration of vaccinations, medications, and treatments as ordered
  • Participates in patient triage, in-person, over the telephone, and via the patient portal. Work with Primary Care Team to assure patients have continuity of care.
  • Manage sessions for family planning services, including outreach, scheduling, and providing family planning nurse visits.
  • Facilitate coordination of patient care including medication management, review of diagnostic test results, follow up on outside services, and completion of patient forms
  • Perform Point-of-Care (POC) and clinical testing as needed
  • Delegate patient care activities with the scope of practice to enhance patient flow
  • Document patient care in the EHR in accordance with health center and nursing standards
  • Implement internal systems to identify when patients requiring additional diagnostics or treatment are due for follow up and contact patients to remind them of the need for services, coordinate with other members of care team and/or external support organizations.

Education and Experience

  • Valid MA Nurse or MA Certification
  • Current Basic Life Support (BLS) certificate
  • Excellent interpersonal skills and ability to connect with patients who are culturally diverse
  • Our health center requires all employees to have the most recent COVID-19 booster and the yearly flu vaccine.
