Nurse Manager – OBAT Program

Under the direction of the Primary Care Provider (PCP), the Nurse Manager coordinates and manages care for patients in the Office-Based Addiction Treatment (OBAT) Program. This role ensures quality, patient-centered care aligned with state and national standards while fostering collaboration among providers, patients, and families. The Nurse Manager supports patient self-management, behavioral change, and continuity of care through evidence-based approaches and effective communication.

Essential Functions

  • Coordinate and manage OBAT patient care under PCP direction, addressing acute, chronic, and preventive needs.

  • Ensure smooth hospital-to-home transitions, reduce readmissions, and support patient engagement and independence.

  • Track and manage patient progress, referrals, and care plans across all care settings.

  • Identify and address barriers to care, including medical, financial, and psychosocial challenges.

  • Collaborate with patients and families on goal setting, education, and chronic disease management.

  • Maintain patient registries, documentation, and outcome tracking to ensure compliance and quality.

  • Adhere to BSAS Standards of Care, including oversight of buprenorphine/naloxone treatment, relapse prevention, and coordination with behavioral health, pharmacy, and primary care.

  • Manage medication processing, refills, and authorizations, ensuring safety and adherence to regulations.

Administrative Responsibilities

  • Supervise and support OBAT nurses and medical assistants.

  • Train new staff, precept students, and serve as a resource across sites.

  • Provide ongoing OBAT education to reduce stigma and improve integration.

  • Represent OBAT at community and organizational events.

  • Maintain cross-training within specialty programs and attend ongoing professional trainings.

Education and Experience

  • MA Registered Nurse License and CPR Certification (AHA required)

  • Excellent communication and organizational skills.

  • Experience with diverse populations preferred.

  • Completion of 8-hour OBAT Nurse Care Management Training and participation in ongoing booster trainings (e.g., relapse prevention, harm reduction, motivational interviewing).
