We are currently hiring a BHI Service Coordinator based in Worcester. The Behavioral Health Integration (BHI) Service Coordinator provides support to the BHI team across all three sites. Responsible for coordinating and facilitating patient-centered, cost-effective care to patients who utilize the BHI services and act as a liaison with medical providers and other health center teams to ensure smooth BHI services. The BHI service coordinator works to streamline BH service, utilizes best practice and supports patients in overcoming Social Determinants of Health. Actively interact with all appropriate outside agencies as needed to foster relationships to support BHI and better serve existing and potentially new patients.
Essential Functions:
- Coordinate and facilitate patient-centered quality care to patients in the Behavioral Health Integration (BHI) program across all three sites.
- Responsible for assisting with interviewing, hiring and training BHI staff.
- Provide services included but not limited to BHI patient screenings, scheduling appointments, educating staff and providers on BHI services and referral processes.
- Provide and connect patients to support for navigating health center and community resources and services when needed.
- Assist patients seen in BHI with post referral follow up.
- Provide direct non-clinical program support and administrative services to the BHI team.
- Help with the management of internal BH waitlists
- Help address patient concerns about BHI services and work to improve BHI service experience.
- Communicate and collaborate with providers and all members of the multidisciplinary health care team as needed to facilitate BHI services and to improve care delivery.
- Facilitate orientation to BHI program services to new BHI clinicians.
- Build relationships with outside BH community-based organizations to help establish referral channels and resources throughout areas that our patients are served.
- Assist the BH leadership with establishing quality measure projects and reviewing patient and visit data.
- Provide support to the BHI team in the development/update of Policies and Procedures for the BHI program and the overall program management.
- Education or Experience in Public Health, Social Work or other related health and human services.
- Experience providing direct social services or similar experience.
