We are hiring a Clinical Care Manager for the Refugee Program in Worcester. This role coordinates care for refugee patients at high risk for health issues, focusing on patient education and self-management to improve outcomes. The Clinical Care Manager ensures care meets national, state, and health center standards. Working within a team-based approach, this role involves coordinating patient care, supporting self-management, using evidence-based methods, participating in patient-focused projects, and employing effective communication with patients.

Essential Functions:

  • Assists the Refugee provider in coordinating culturally sensitive care for refugee patients, covering acute and chronic illness management, immunizations, lifestyle education, and preventive care across settings.
  • Completes the nursing process, collaborates with a multidisciplinary team, performs treatments, and provides physical, emotional, and educational support.
  • Works with the clinical team to manage patient care, addressing health, financial, and psychosocial barriers.
  • Communicates patient needs and care plans with the PCP, team, and family. Supports refugees in achieving self-sufficiency and involves families in care decisions.
  • Utilizes community resources, assists in action planning for self-management, and uses behavioral strategies to promote healthy behaviors and independence in healthcare navigation.

Required Qualifications:

  • Registered Nurse License in MA
  • Current BLS CPR certification
  • Excellent interpersonal skills are necessary
  • Excellent Verbal and Written Communication Skills
  • Experience working with multicultural, multiethnic populations.
