The OBAT (Office Based Addiction Treatment) Coordinator based in Milford, MA, will support the team by managing state and federal reporting, maintaining patient registries, and coordinating care for patients with substance use disorders. They address Social Determinants of Health and collaborate with external agencies to enhance OBAT services and reach more patients.

Salary for this position: $24.00-28.00/Hour 

Essential Functions:
  • Coordinate patient-centered, cost-effective care for OBAT patients.
  • Hire, train, and supervise nonclinical OBAT staff.
  • Conduct patient screenings, schedule appointments, and provide social support navigation, including benefits applications and referrals.
  • Offer nonclinical administrative support to the OBAT team.
  • Collaborate with providers and the healthcare team to manage patient registries, appointments, and outreach.
  • Complete required paperwork for OBAT patients.
  • Facilitate new patient orientation within a month of intake.
  • Build relationships with community organizations to enhance service coordination and referrals.
  • Support OBAT team with grant tracking, reporting, and external programming.
  • Assist in updating OBAT policies and procedures.
  • Perform additional duties as assigned, including attending meetings and trainings.
Required Qualifications:
  • Education in Public Health, Social Work or other related health and human services.
  • Experience providing direct social services or similar experience.
